1. My pledge regarding your health information

I am committed to protecting your protected health information (PHI). I create and maintain records of the care and services you receive in order to provide quality care and comply with legal requirements.

2. Uses and disclosures without authorization

Your PHI may be used or disclosed for treatment, payment, and health care operations, including administrative services through Headway, as required by law, for public safety, legal proceedings, and appointment reminders.

3. Uses requiring written authorization

Psychotherapy notes, marketing, or sale of PHI require your written authorization unless otherwise permitted by law.

4. Your rights

You have the right to:

• Inspect and obtain copies of your record (excluding psychotherapy notes)
• Request amendments to your record
• Request restrictions on certain uses and disclosures
• Request confidential communications
• Receive a copy of this notice

5. Email and text communication

Email and text communication are not guaranteed to be secure. Do not use these methods for emergencies.

6. Complaints

You may file a complaint with me or with the U.S. Department of Health and Human Services without retaliation.
